Band outward turn exercise ยท shoulder pain

The band outward turn exercise builds the rotator cuff, the four small muscles keeping the ball of your shoulder centered.

The outward turn with a band is a home shoulder exercise: elbow bent 90 degrees and pinned against your ribs, you rotate your hand outward against the band, pause 2 seconds, return slowly, and repeat 10 times.

Line drawing of the outward turn with band exercise. The pale figure is the starting position, the dark figure is the finish, and the orange arrow shows the movement.
Light gray: where you begin. Black: where you end up. The orange arrow traces the motion.

Skip this one unless it is marked on the home program your physician gave you. Unless your instructions say otherwise, do your marked exercises twice a day, every day, and tick off each session in your log.

Step by step: the outward turn with band

  1. Set up: Wedge a rolled hand towel against your ribs, under the elbow. With the elbow bent 90 degrees, grip the band, which is anchored toward your opposite side.
  2. Move: Without letting the elbow leave the towel, rotate the forearm outward against the band, then let it return under control.
  3. Count and hold: Pause 2 seconds. Repeat 10 times. Twice a day.

Why the outward turn with band matters after a procedure

The rotator cuff holds the ball of the upper arm centered in a shallow socket. When it weakens, the ball drifts upward and nips the tissue overhead every time you lift your arm. Training outward rotation is one of the most direct ways to recenter the joint, whether the pain came from wear, a tear or an injury. Patients treated with PRP or other orthobiologics for the shoulder build on that result here.

Which mistakes undo it?

  • Letting the elbow drift away from your body.
  • Letting the band snap back.
  • Twisting your trunk instead of rotating the arm.

When to stop for the day

Expect a mild stretch, some effort, and maybe a little soreness the next morning. Quit for the day and phone (314) 886-5620 if you get:

  • Numbness, tingling or weakness that was not there before
  • A sharp, stabbing pain
  • Pain that rises 2 points or more above your starting number and does not settle within an hour
  • Lightheadedness, or your vision changing

Fainting, chest pain or a struggle to breathe: dial 911, not our office.

What to remember

  1. The elbow stays pinned to the towel.
  2. Out slowly, back slower.
  3. Only the forearm swings.

Last step. Show us you have it: Right here in the clinic, demonstrate one outward turn with the towel still in place. Unsure about any step? We run through it with you again next time.

Why bring your exercise log?

The log is how we tell the difference between an exercise that is not working and an exercise that is not being done. At each visit your technician counts the sessions you checked off, notes your daily pain number, and watches you do one exercise to correct your form before small errors become habits.
