Quad set exercise ยท knee pain
The quad set exercise switches the thigh muscle back on after knee pain has quietly shut it off.
The quad set, or thigh squeeze, is a home knee exercise: leg out straight over a rolled towel, you tighten the front of the thigh to press the knee down, hold five seconds, 10 times.

Skip this one unless it is marked on the home program your physician gave you. Unless your instructions say otherwise, do your marked exercises twice a day, every day, and tick off each session in your log.
Step by step: the thigh squeeze (quad set)
- Set up: Sit or lie back, leg extended, with a towel roll tucked behind the knee.
- Move: Contract the large muscle along the front of the thigh, driving the back of the knee into the towel. The kneecap should slide up slightly.
- Count and hold: Hold 5 seconds. Repeat 10 times. Twice a day.
Why the thigh squeeze (quad set) matters after a procedure
Knee pain and swelling send a signal that partly blocks the quadriceps; doctors call it arthrogenic muscle inhibition. Within days the thigh weakens, and once it does, stairs and curbs become places where the knee gives out. Whether your knee was treated with a genicular nerve block, an image-guided injection or PRP, this is the first step in getting the muscle back online.
Which mistakes undo it?
- Holding your breath during the squeeze.
- Lifting the heel instead of pushing the knee down.
- Releasing early, before five seconds are up.
When to stop for the day
Expect a mild stretch, some effort, and maybe a little soreness the next morning. Quit for the day and phone (314) 886-5620 if you get:
- Numbness, tingling or weakness that was not there before
- A sharp, stabbing pain
- Pain that rises 2 points or more above your starting number and does not settle within an hour
- Lightheadedness, or your vision changing
Fainting, chest pain or a struggle to breathe: dial 911, not our office.
What to remember
- The knee pushes down into the towel.
- Watch the kneecap rise. That means it is working.
- A full 5-second hold every time.
Last step. Show us you have it: Right here in the clinic, demonstrate one squeeze and point to the kneecap as it moves. Unsure about any step? We run through it with you again next time.
Why bring your exercise log?
The log is how we tell the difference between an exercise that is not working and an exercise that is not being done. At each visit your technician counts the sessions you checked off, notes your daily pain number, and watches you do one exercise to correct your form before small errors become habits.
